The Hearts4home Personal Care Services are designed to assist our clients with activities of daily living also known as ADL's in their home.
This service includes:
Dressing: providing assistance with changing and choosing clothing to maintain good healthy hygiene.
Bathing: assistance with transfers to shower chair, giving verbal prompts to complete task, encourage clients to engage actively in self-care activities and making sure modesty and dignity are maintained at all times.
Eating: Make sure client has all utensils needed for meals. offer to cut food if client is having difficulty, encourage healthy options while maintaining a healthy space.
Toileting/Incontinence Care: assist with sitting and standing, help client maintain proper hygiene, give reminders and verbal prompts to visit the restroom, help client prevent the breakdown of skin and encourage frequent changes in briefs and pads.
Medication Reminders: create a schedule or set a timer to remind client to take medication. Assist with refilling prescriptions and report any medication problems.
Mobility: Assist with walker, wheel chair or cane. Caregivers are trained to help clients with assisted devices and keep a safe environment.

When a loved one is being discharged from the hospital, it can be an overwhelming and confusing experience with a lot of information to take in.
Let "Hearts4home" prepare your loved one for their transition back home from a hospital stay, an assisted living facility or rehabilitation center.
Our trained and certified staff can help make sure we adhere to all discharge instructions and make sure they are followed correctly.
Our goal is to lift the burden off the families shoulders so they can focus on what's important knowing that their loved one is being taken care of properly.

Don't own a vehicle? Or you simply cannot drive yourself? The "Hearts4home" team is ready to assist you with your transportation needs.
As a part of our service we can provide transportation to doctors appointments, grocery shopping, visits with friends and relatives, religious functions and entertainment events.
